چکیده

Microembolization resulting from platelet aggregation, air embolization and infusion of particulate material with stored and coronary suction blood has been implicated in the pathogenesis of certain complications following heart surgery'-s. In previous studies we have noted that particulate microembolization is reduced and platelet function is preserved following cardiopulmonary bypass with a membrane oxygenator when compared to a bubble oxygenator6 , 7 • However, although microembolization resulting from blood oxygenation is reduced with membrane oxygenation, considerable particulate material was found in blood returning to the patient through the cardiotomy return system. In the present study, the filtration characteristics of various devices used in the cardiotomy reservoir system were evaluated. METHODS

A prospective randomised comparison of cardiotomy suction and cell saver for recycling shed blood during cardiac surgery.

OBJECTIVE Post-operative neuropsychological complications correlate with intra-operative microemboli in the middle cerebral artery. When severe neurological complications follow cardiac surgery, diffuse cerebral fat emboli are present at autopsy. Recycling shed blood with cardiotomy suction is an important source of cerebral fat microemboli. A cell saver may reduce this. METHODS Twenty patien...

An ultrasonic analysis of the comparative efficiency of various cardiotomy reservoirs and micropore blood filters.

The ability of 12 commercially available cardiotomy reservoirs to remove bubbles from aspirated blood was investigated by means of a simulated cardiopulmonary bypass circuit and an ultrasonic microbubble detector. Performance varied considerably. The number of gaseous microemboli remaining after passage of blood through the reservoir was reduced by (a) holding the blood in the reservoir, (b) re...

The cardiotomy trial: a randomized, double-blind study to assess the effect of processing of shed blood during cardiopulmonary bypass on transfusion and neurocognitive function.

BACKGROUND Reinfusion of unprocessed cardiotomy blood during cardiac surgery can introduce particulate material into the cardiopulmonary bypass circuit, which may contribute to postoperative cognitive dysfunction. On the other hand, processing of this blood by centrifugation and filtration removes coagulation factors and may potentially contribute to coagulopathy. We sought to evaluate the effe...

An Evaluation of A Modified Cobe Cardiotomy Reservoir

Even though the unit was in use for approximately 8 years with no apparent difficulties, awareness of the benefits of blood filtration were becoming obvious. Many groups across the country were beginning, along with the Cleveland Clinic, to investigate the benefits of blood filtration in all phases of patient care including open heart surgery. This investigation was prompted by the high inciden...

Clinical evaluation of a new fat removal filter during cardiac surgery.

OBJECTIVES Fat microemboli are generated during cardiac surgery that are associated with post-operative organ injury. Recently, a fat removal filter has been developed, based on a polyester leukocyte depletion filter. However, the efficacy of such a filter in a clinical setting is unknown. In this study we tested the efficacy of this filter. METHODS Coronary artery bypass patients were random...
